We are concerned that there have been multiple instances where such reports have not been acted on.","additional_details":"In one such instance, multiple prevention of future deaths reports were submitted to Government Departments to warn them about a harmful online suicide forum which promoted the sale of poison. We believe the Government did not do enough to act on this.
