Closed NHS £8 Billion by 2020. A full and honest answer; where will it come from?

We, the undersigned, would like a full, honest and detailed account of where the promised extra £8 Billion for the NHS will come from. The PM regularly referred to his 'balanced plan' for the economy during the General Election but failed to answer any question about where it will come from.
