Too many deaths or extended suffering caused by erroneous comments in medical records. There is also too much focus on psychology as a cop-out diagnosis.

When the patient is later found to have a serious physical illness, the error and misdiagnosis remains in the medical records, along with any discourteous remarks.
The patient is not empowered and can't get the entry removed, only blocked.
