There should be an extra box at the foot of all voting slips for national and local elections and referenda, under the list of candidates, which says "None of the above". If more than 50 % of the votes cast tick this box, then there must be a re-election, and none of the previous candidates are allowed to stand. I believe that this already happens somewhere in the USA.
