Introduce swingeing fines for utility companies that:
dig holes, then leave them abandoned with no work being carried out;
overrun completion dates for the work;
don't clear up cones and barriers as soon as work is finished.

Close monitoring and heavy fines are the only way to change their behaviour and minimize disruption.
