In view of serious case reviews (SCR) most reports conclude a combination of poor information sharing, poor communication, including IT systems, which seem to actively prevent information sharing within professions, as well as poor practice leads to missed opportunities.

Currently professionals are using different systems and are often pressed for time when communicating. Logging on to ONE IT system and recording interventions amongst the professionals would be such an advantage towards inter professional collaboration,communication and minimising risk.
