Closed Transition from paediatric to adult care

There is an urgent need for the Department of Health to look into the serious situation that effects a patient with a life long chronic disorder when they reach the age of 16.

Paediatrics wash their hands at age 16 and adult services don't want to know either.

For a child that has had a lifelong illness those 2 years are just a huge frustrating hole in the system that causes upset and frustration to an already stressed family.

This HAS to be investigated and discussed by the Govenment.
