From experience i think the questions are more aimed at people with physical disabilities and mental retardations rather than mental health issues.

Can you prepare food?
Can you wash yourself?

2. How do you feel when you go outside?
3. Do you have times when you neglect your hygiene?
4. Have you ever had addiction problems?
5. How are you at handling your finances?
6. How is your memory?
7. How often do you experience a symptom? Ect ect...
