All medical professionals should be legally required to report any unintended or unexpected incident, including mistakes, which could have, or did, lead to harm for one or more patients. No matter how detrimental they may be.

There is no law requiring that incidents that lead to the harm of patients must be reported by the medical professionals involved. This must change.
