Fund an improved mental health service enabling a seamless transition from acute crisis care (Home Treatment/Rapid Response) and community support. Establish a statutory "Continuity of Care" guarantee with a safety/care plan before a patient is discharged from intensive services.
With the additional funding, government should mandate that every mental health services user has a single point of contact responsible for coordinating referrals and responding to clinical deterioration. We believe the GP and 111 route to get help currently lead to a "black hole" in community care, and patients waiting for months to receive a risk assessment, and are only able to access online group sessions, regardless of their condition or state of deterioration as a result. Consistent and individualised support should be funded and prioritised.
