We are calling for a national statutory Health & Needs Passport, available in both digital and paper formats. The digital version would be accessible across the NHS, and the paper version would provide a quick access summary with a QR code linking to the full medical record.
Parents of disabled children and young adults are repeatedly asked to retell their child’s full medical history at every appointment. This can be exhausting & retraumatising, and lead to inconsistent care. This new system should ensure professionals have immediate, accurate information — reducing repetition and improving care. No parent should have to relive their child’s entire medical journey at each & every appointment.
