No statutory body examines failures across the whole criminal legal process spanning police, prosecutors and courts. This includes witness contamination or Section 28 of the Youth Justice and Criminal Evidence Act 1999 breaches.

We want an independent body to establish facts, identify failings and refer misconduct to relevant authorities.

When failures span several organisations, no independent body examines the whole process. This can leave witness contamination, Section 28 breaches, false evidence, missing records and safeguarding failures across the criminal legal process without proper investigation. The proposed body could establish facts, identify systemic failings, refer suspected misconduct and recommend improvements.
