Require NHS trusts in England to maintain publicly accessible, searchable registers of approved clinical and patient-safety policies, including superseded versions and their effective dates, subject to proportionate confidentiality and security safeguards.

Patients and bereaved families should be able to identify the clinical policies and standards that applied to NHS care.

Despite Freedom of Information duties, clinical policies and historical versions can be difficult to access.

Families investigating serious harm or death can face lengthy requests, refusals and appeals to establish which procedures were in force.

Routine publication would improve transparency, reduce costs, support independent scrutiny and strengthen NHS accountability.
